Glass & Screen Work Order specially designed for new fabrication. Form is consecutively numbered in three positions when we are printing your personalization. You can specify a starting number of we will start numbering at 01001 and we will continue numbering on reorders. Price shown is your NET price and includes your store personalization with your store program or custom logo in black ink ($35 one time charge for custom logos).

• Jumbo numbering for easy identification.

• Bottom stub holds two Master Labels. Remove liner on back side to expose adhesive. Two labels have special repositionable/removable adhesive (will not stick to dirt or dust) and leaves no residue on most surfaces. The master and secondary labels organize and identify all of the articles.

- Secondary Labels -

Personalized labels in strips of eight and shrinkwrapped. Use Secondary Labels when more that two articles are being fabricated and/or repaired. Personalized Secondary Labels are packaged 1,024 to a package and are sold separately for $122.00 per lot. These lables look different and are smaller than the master labels so that one can distinguish immediately between master and secondary lables. The article I.D. number needs to be hand written in on secondary labels. Article I.D. number then ties multiple pieces together. These labels are 2 1/2" x 5/16" and have the same repositionable adhesive as the master label.

For sturdy protection of your paperwork you can use Transparent Envelopes (Mfg. No. HDWE 8) that are specially sized for this Glass & Screen Work Order (transparent envelopes must be ordered separately).

• Oversized giant/bold numbering for easy identification.
• Form is designed to fill out the face side and the back side. When filling out the face side, information will carbonize onto plies 2 & 3. When ready for costing on reverse side, information will carbonize onto back of ply 1 (store copy). Customer comments and instructions to technician will then be on the back of both the work order and the store copy.
• Stub #1 is your Reference ID Tag. This ID tag gets attached to the repaired items. Oversized numbering identifies and organizes your orders.
• 2 ways to apply top stub (Reference ID Tag) to item being repaired - Use punch hole at left end to stub and using a Twist Tie, attach to item being repaired, or apply (separate) Display Label as shown on back side of top stub.
• Order Includes 500 Display Label per 1,000 forms ordered. Additional Display Labels must be ordered separately.
• For multiple article repairs, you have a choice of multiple Screen & Window Repair forms or multiple Secondary Labels.

For sturdy protection of your paperwork, you can use the specially sized (reusable) Transparent Double Pocket Envelope (Mfg. No. HDWE 2) that are specially sized for this Screen & Window Repair Form, and wire this to the damaged screens and/or glass after repair, then apply the master and secondary labels (transparent envelopes, wire and secondary labels are sold separately).

3 Ply Personalized Equipment And Small Engine Repair Carbonless Service Order Form

Equipment & Small Engine service order Form with I.D. tag, priority card, claim check and store card at the bottom of ply 3. Ply 3 is a green tag stock work order with room on the reverse side for service department entries. Plies 1 & 2 are 8 1/2" x 9 5/8" and ply 3 is 8 1/2" x 12 5/8". Forms are pre-numbered during production so you need not specify numbering. You will receive the next sequential numbering in stock. • Over sized numbering for easy identification. This form is numbered in four locations.
• Bottom of ply 3 (Work Order - Green Tag Stock) extends and includes Service Order I.D. Tag, Priority Card, Claim Check and Store Card. On Store Card we will imprint the store Business Card copy (as illustrated) unless you instruct us otherwise. This card is for you to use your creativity. We will print any copy that you desire.
• The I.D. Tag has reinforced punch holes for you to attach to the item being repaired (wire must be ordered separately ~ see below). This I.D. Tag will have your store personalization imprinted on it.
• The Priority Card is used for reference and aids in prioritizing your repair work. There is a punch hole if you want to hang these cards for visual planning.

3 Ply Personalized Universal Repair Order Form With Oversized Numbering

Ply 1 (claim check) has your store personalization and is give to customers at the time the article(s) to be repaired is dropped off. Ply 2 (store copy) is customer's paid receipt or could be used as permanent store copy, accounting, auditing, department copy, etc. Ply 3 (repair work order copy) goes with article(s) to your service department. This copy has all the information and instructions to be followed for repair, and to record on reverse side all work performed and costs. Keep as permanent record for later reference. Forms are prenumbered during production so you need not specify numbering. You will receive the next sequential numbering in stock.
